EVENING APPOINTMENTS: Yosemite Pet Hospital is also open some evenings, to help accommodate a busy work schedule. Please call for available evening appointment times.
CLIENT INFORMATION SHEETS: If this is your first visit to Yosemite Pet Hospital, or if you are bringing a new pet to the Hospital, we will require completion of a Client Information Sheet. To help save time, you can download this form and fill it out prior to visiting our offices. Simply bring this form with you when you come in. You can also fax it to our offices in advance.

"DAY STAY" APPOINTMENTS: We have early morning "Drop Offs" available for clients who may be unavailable during the daytime. You may make an appointment, and arrange to drop off your pet at our Hospital in the morning. We will examine your pet, and contact you to discuss and approve a treatment plan. Please note that a nominal fee is applied for the Day Care of your pet.
We require a "Day Stay Consent Form" for pets who will be dropped off in the morning. To help save time you can download this form and fill it out prior to visiting our office. Simply bring this with you when you drop off your pet.
